A person with Locked In syndrome is awake and aware but is unable to respond because they are paralysed and unable to speak. This is usually as a result of damage to the brainstem and there is no loss of cognitive function.
 
Typically the only movement they can make is by blinking or up and down movements with the eyes, though some people may have very limited movement of the head.
